Forms and documents
When applying for regular residential service with an existing meter, you are required to submit the following:
- Water Service Application
- Customer Service Agreement
- We need a copy of your driver’s license, which can be done at the office
- Proof of ownership, such as a warranty deed. If you are renting your residence, we require a Rental Agreement signed by you and the landlord.
Also, the non-refundable connection fee is required with the paperwork.

For Temporary Service options of 3 days or 30 days, we require a Temporary Water Service Application.
If you elect to enroll in ACH Bank Draft, your payment will be drafted between the 6th and the 8th of each month. You will still receive a bill every month.
If you would like to donate monthly to the Sardis City Volunteer Fire Department, please print out the flyer and bring by the office or send in with your payment or application.
We do not provide garbage pickup services. If you live in the city limits, call Sardis City Town Hall at 256-593-6492 for the garbage information. If you live outside the city limits, try Meridian or Republic Services.
